We are assisting a Local Authority in the appointment of an experienced Health and Social Care Officer to join their Adults Social Care Team in Nottingham. As a Health and Social Care Officer provide advice, triage and coordinate information to enable decision making and risk management on behalf of citizens requesting / requiring access to social care and health services, and to navigate citizens through to the right services based on an assessment of their needs. Key Responsibilities: - • Coordinate telephone calls, enquiries and referrals from citizens, vulnerable adults, families and carers and to manage these in an effective, timely and customer focused way. • Deliver an agreed level of customer service that reflects the vision, values, aims and objectives of Nottingham City Council and partners. • Manage specialist or escalated calls from Health and Social Care Support Coordinators, Health and Social Care Support Advisors or other colleagues. • To have an understanding and know ...
