POSITION SUMMARY, Community Reintegration Worker The Community Reintegration Worker (CRW) works directly with clients, providing frontline advocacy, counselling and crisis intervention services while maintaining high levels of respect and trust. The incumbent takes an active role in assisting clients to reintegrate into the community by facilitating connections with external agencies. DUTIES & RESPONSIBILITIES • Provide advocacy, short-term counselling and crisis intervention for clients residing in, visiting or telephoning the facility, including: • Participating in institutional visits to potential clients as required. • Provide clients with referral services to external community resources when appropriate. • Administer frontline service functions including but not limited to: • Keeping up-to-date client records; • Managing a caseload • Writing monthly progress summaries • Monitoring PBC conditions. • Provide administrative support to the agency as tasked by the Executive Director ...
